The implementation of the project is divided into six work packages (WP). Work packages 1 and 2 will create the strategic framework for the transnational network and will specify the structure and organisation of the co-operation. In work packages 4 and 6, technical solutions and standards for transnational eHealth applications will be developed and implemented in the field of eHealth solutions. Work package 3 will focus on strategies improving the acceptance of eHealth technologies, and work package 5 will focus on eHealth technology education.
WP1 Project management and administration
Expected outcome: High level of project quality through effective project management and administration.
WP2 Communication and information
Expected outcome: Effective internal and external communication and dissemination of project activities and interim results.
WP3 Strategies to improve the social capacity of citizens and medical professionals to utilise eHealth technologies
Expected outcome: Overview of transferable regional strategies and policies which address the deployment of ICT solutions for elderly people with chronic diseases, incl. improvement of ICT skills among the elderly, to be used for awareness building and discussions between stakeholders from policy, health sector and industry. Transnational analysis of the ageing population’s impact on health care systems and chronic diseases in the Baltic Sea Region to be used as the basis for transnational discussions between stakeholders from policy, health sector and industry on the potential of eHealth utilisation
WP4 Empowerment of citizens with chronic heart disease through lifelong learning and self-monitoring
Expected outcome: Self-monitoring and self-learning system used by 400 pilot programme patients with chronic heart failure and medical professionals giving direct feedback on the health data in the pilot regions. The services will be transferred and offered to organisations and the general public in the Baltic Sea Region after project finalisation.
WP5 Education for health care professionals and citizens with chronic diseases to utilise eHealth technology
Expected outcome: Education courses on the utilisation of eHealth technologies for medical professionals and citizens with chronic diseases and elderly people (3 modules), to be used during the project by citizens and medical professionals in the pilot programme regions, and subsequently offered to the general public in the Baltic Sea Region.
WP6 Enabling mobile citizens with chronic diseases to document health data in a multi-lingual personal health portal
Expected outcome: Multi-lingual personal health portal with international comprehensible medical information used by 400 patients from the WP4 pilot programme. The health portal and related self-monitoring services will be transferred and offered to organisations and the general public in the Baltic Sea Region after project finalisation.
